Johns Hopkins After-Care Clinic
The Johns Hopkins Hospital has established a pilot program to provide care for patients discharged from the hospital or Emergency Department who need rapid follow-up but cannot get timely appointments from their primary care clinic or do not have a "medical home." Beginning April 1, 2015, the Johns Hopkins After-Care Clinic will operate in the Outpatient Center on Mondays, Wednesdays and Thursdays from 4:30 to 7 p.m. in existing space on the seventh floor. Medical leadership includes Rosalyn Stewart and Arjun Chanmugam.
